We provide complete
out patient mental health and substance abuse services.
These services include:
- Assessment and diagnosis
- DUI assessment and classes
- Individual, family, and group therapy
- Evaluation for medication
- Medication management
- Skills development
- Case management
- Intensive substance abuse outpatient services
- Psychological testing
- Short-term treatment for children, youth, and adults suffering life crisis situations
- Ongoing treatment for people with chemical dependancy
- Ongoing treatment for people with biological or severe mental illness

At Four Corners You Can Expect
Respect and Dignity
You will be treated with respect and dignity and without discrimination by
Four Corners staff. Every effort will be made to help you mobilize your support
system and to avoid unnecessary hospitalization or out of community placement.
Effective Treatment
You will receive effective treatment by skilled prfessionals. Your services
will start with a thorough assessment by a mental health therapist to diagnose
your problems and to review treatment options. Together, you will negotiate
a course of treatment that meets your needs and relies on state-of-the-art
approaches. A substance abuse professional may help the therapist with the
assessment if your problems are substance abuse related.
Confidentiality
Your confidentiality is a top priority for us. Your relationship with Four
Corners is completely confidential.
Convenience
Four Corners has clinics in Carbon, Emery, and Grand Counties and offers Intensive
Outpatient Programs for substance abuse care as well as an array of mental
health services. Four Corner's staff also provides services on a regular basis
in East Carbon and Green River.
Information for Medicaid Clients
Four Corners can't pay for any services unless we approve them first.
Call us before you get any kind of mental health care.
Your Responsibilities
It is your job as a Medicaid consumer to:
- Always show your Medicaid card
- Always keep appointments or call, cancel, or reschedule
- Follow treatment instructions
- Ask for help if you need it
- Talk to your therapist, the local clinic supervisor, or the Four Corners Administrative Office (435-637-7200)
- Ask for a copy of the Four Corners Grievance Handout
- Call the Medicaid Hotline at 1-800-662-9651 to discuss a concern with your mental health treatment.
Need a ride to your mental health appointment?
Help with transportation to your mental health appointments is also available.
You may request services in a preferred language other than English.
Four Corners will provide a translator if you say you need one.
