Community Outreach Team
The community outreach team of nurses, case managers and clinicians provides comprehensive and intensive community-based treatment to persons with severe and persistent mental illnesses. A multidisciplinary team, who function as a mobile health unit providing treatment, rehabilitation and support, provides services. This service is designed for individuals who have severe and intractable symptoms of a psychiatric disorder who need frequent contacts and for whom clubhouse or office based services have not been effective.
- Case management services to assist individuals in obtaining housing, entitlements, education, vocational support and substance abuse services.
- Skills development services to help individuals build basic living skills such as money management, food preparation, housekeeping, shopping, grooming and other interpersonal and social skills.
- Budgetary services help individuals who need help in planning and following a budget.
- Psychiatrists and registered nurses provide medication management services. Individuals receive education about illnesses and the medications used to treat them. Medications are delivered to many clients to increase compliance and consistency.
- Therapists assist individuals with supportive therapy and setting and achieving personal goals.
Who is eligible: Individuals residing in Carbon or Emery Counties referred by Four Corners mental health therapists.
